#16 is interesting because no one really knows about it. Myself and a former colleagu did a test on it about 4 years ago and published our results which conculded what you are saying. Since then I’ve been careful to follow this rule. The only issue is that often times using the exact kw does not “work” for navigation anchor texts. But with a little CSS trickery one can get the code for the nav bar to be lower in the code, prioritizing contextual links. I’ve also seen sites add links to 3-5 specific and important internal pages with keyword rich anchor texts, at the very top of the page in order to get those important internal links to be indexed first.

This is excellent and should be intuitive for marketers (and SEO pros are marketers!) but we often take the short cut and neglect critical details. What would also reinforce the strategy is way of providing solid projections for SEO (these could be based on industry trends and statistics). Clients now ask for ways to calculate ROI and they need numbers to get budget approvals. Increase in traffic by X,  increase in qualified traffic and leads, conversions etc, some way of quatifying the expected return.
Having a different description meta tag for each page helps both users and Google, especially in searches where users may bring up multiple pages on your domain (for example, searches using the site: operator). If your site has thousands or even millions of pages, hand-crafting description meta tags probably isn't feasible. In this case, you could automatically generate description meta tags based on each page's content.
AdWords and Facebook are further vehicles for reaching the appropriate audiences with more refined messaging. I think it's important to create personas for your current visitors and the type of visitors you want to attract. It might be valuable to create personas of those you don't want to attract, to keep in the back of your mind as your content and advertising calendar is being built following the delivery of your overall strategy.
On another note, we recently went through this same process with an entire site redesign.  The executive team demanded we cut out over 75% of the pages on our site because they were useless to the visitor.  It's been 60 days since the launch of the new site and I've been able to still increase rankings, long-tail keywords, and even organic traffic.  It took a little bit of a "cowboy" mentality to get some simple things done (like using 301's instead of blocking the old content with robots.txt!).  I predicted we would lose a lot of our long tail keywords...but we haven't....yet!

Like many SEOs, I was hired with one vague responsibility: to set up an SEO program and achieve results. Quick question: I am beyond passionate about a niche (UFOs, extraterrestrials, free energy) and know in my bones that an authority site is a long term opportunity. The first relates to internal link structure. I’ve made the mistake you say you’ve seen so often. I have a primary keyword and have used that keyword in the main navigation, linked to a page optimized for that keyword. But I’ve also got a bunch of contextual links in posts pointing to that page, usually with the keyword in the anchor text. I now understand that those internal links aren’t helping much, at least from an SEO perspective. Am I better to remove that keyword and direct link from the menu and simply link the page from multiple posts and pages within the site. Or will I get better results leaving it in the main menu and changing the contextual links in the posts to point to a related page with a different keyword?
